How do I request a copy of my HESI transcript?

Click here to download the HESI transcript request form and follow these steps:

Step 1: Complete the request form and mail to transcripts@elsevier.com or send via fax to 713-346-6975.
Step 2: After submitting your completed request form, please call Elsevier 1-800-950-2728 Option 4 with your credit card information within 3 to 5 business days after you have submitted your request form or your transcript request will be discarded.

NOTE: Your credit card will be charged a $15.00 processing fee.
